At Hamm Health Professionals LLC, we provide a wide range of lab testing services designed to support your health and wellness goals—whether you're just getting started or managing ongoing concerns. We’ve partnered with Rupa Health to coordinate lab testing, including local phlebotomy services for added convenience. If you’re exploring a weight loss program, we also provide custom lab bundles and monthly follow-ups to help you track your progress and stay on course.
Checkout through our Partner Rupa Labs and then send us a text at 803-220-4075 to set up an appointment. A photo of your ID must be texted before consultation to verify identity.

Our custom lab bundles make it easy to get the answers you need with a provider-guided approach. Each bundle includes an included provider fee for a telehealth consultation. We recommend scheduling appointments 3 weeks out to give the lab adequate time to process results.

Hello, I’m Monica Hamm, a dedicated Family Nurse Practitioner licensed in Arizona, California, Delaware, Florida, Georgia, Idaho, Iowa, Kansas, Kentucky, Montana, New Hampshire, New Mexico, North Dakota, Oklahoma, South Carolina, Tennessee, Utah, Virginia, and Wyoming.
With over a decade of nursing experience, I’m passionate about making high-quality healthcare accessible and supportive. My career began as an LPN and evolved through continued education, leading to a Summa Cum Laude graduation from South College. My background includes extensive experience in wound care, hospice, rehabilitation, and patient care coordination as both an MDS Coordinator and Wound Care Nurse.
At Hamm Health Professionals LLC, I offer telehealth-based care for a wide range of primary and urgent care concerns. My specialties include weight loss management, urgent and primary care, lab interpretation, hormonal testing, functional medicine, and more. I am certified in both telehealth and advanced life support, and I focus on compassionate, patient-centered care by truly listening and creating treatment plans that align with your lifestyle and needs.
To support your wellness journey, I also provide comprehensive lab testing—both bundled packages and standalone services—with a telehealth consult included in each bundle. Popular options include routine labs, weight loss panels, hormone testing, stool analysis, menopause assessments, and full functional health panels. Through my partnership with Rupa Health, patients also have access to local phlebotomy services for added convenience.
We provide custom weight loss consultations in partnership with Sesame, with personalized care plans and monthly medications delivered directly to your home through our partnership with Sesame. For those pursuing weight loss, I offer customized lab bundles, initial evaluations, and ongoing monthly follow-ups to track your progress and adjust your plan as needed.
Outside of my clinical work, I proudly volunteer with the Women’s American Legion Auxiliary, supporting veterans and their families. Payment
All telehealth services must be paid in full prior to the start of your tele-visit. Appointments will not begin until payment has been received and confirmed. Failure to complete payment may result in cancellation of your appointment. 12-hour notice is required for cancelation.
All patients are required to present a valid government-issued ID at the start of their appointment. The name on the ID must match the name used during registration. Failure to verify identity may result in cancellation of the visit without refund.
Emergency
Scope of Care
Telemedicine is intended for non-emergency, minor urgent, and primary care concerns. It is not appropriate for life-threatening, rapidly progressing, or emergent conditions. If you are experiencing a medical emergency, call 911 or go to your nearest emergency room immediately.

Telehealth services are only available to individuals who are physically located in the following states at the time of their appointment:
